Step 1 – Getting the Application

Step 2: Completing the Application
Note: Applications will NOT be reviewed by our Medical Team until all of the below items have been received.
All information must be received by April 1st. For Family Weekend Sessions, all information must be received at least four weeks prior to the camp requested.
- Application
- Parent, guardian or caregiver please complete online application
- Camper Medial Form – 2 pages
- Camper Medical Form must be completed and signed by your child’s health care provider
(Medical Form is not required for Autism day camps)
- Immunization Records
- Please include an up-to-date copy of immunization records for everyone under age 18 (Immunization Records are not required for Autism day camps. All attendees will be required to complete a brief symptom screen upon arrival to camp. Please do not come onto camp grounds if you are experiencing any medical symptoms.)

- Special Instructions for Hemophilia With Inhibitors Family Weekend: This Disease Specific form must be submitted with the above information Bleeding Disorders form. Please note that all information submitted will be shared with Comprehensive Health Education Services, LLC.
Step 3 – Sending the Application
Electronic applications are submitted online. Paper applications should be faxed, emailed, or mailed.

Step 4 – Processing
- You will receive a phone call or email informing you that your complete application has been received or if any further documentation is required
- If your child/family is not eligible based on our criteria, you will receive a phone call or email from a member of our Selection Criteria Team
Step 5 – Acceptance
- Acceptance notifications for family weekends will be sent 2-4 weeks prior to your requested session.
Acceptance notifications for summer camp will be sent no later than four weeks prior to your requested session.
